Description/Mechanism

Reteplase is a DNA recombinant human tissue-type plasminogen activator (TPA) which acts as a catalyst in the conversion of plasminogen to plasmin.

Plasmin is responsible for degrading the fibrin mesh-like blood clot.

Within minutes

Duration

half-life between 13 and 16 minutes.

Indications

Thrombolysis management of actue MI (not approved for use in ischemic stroke).

Contraindications

Active internal bleeding

History of CVA

Recent reuologic surgery or trauma

Bleeding disorders

Severe uncontrolled hypertension.

Caution in pregnancy

Adverse Reactions

Hemorrhage

Anemia

Drug Interactions

Aspirin and heparin may increase risk of bleeding. Both are typically given concurrently to increase thrombolytic properties.

Supplied

A complete administration kit contains 2 10 unit vials of medication, diluent, syringe, needle and dispensing pin.

Dose/Administration

Adult

Administer as IV bolus of 10 units injected over 2 minutes, repeat in 30 minutes.

Pediatric

Not recommended

Special Consideration

Pregnancy Safety: Category C

Reconstitute with sterile water according to manufacturer's recommendations.
